ANDOVER CENTRAL SCHOOL REGISTRATION FORMS

Please find below registration forms for students entering pre-k, kindergarten, elementary grades 1-6 and Jr/Sr high school grades 7-12.

When you come to register your students please bring the following along with these forms.

  1. Proof of residency (utility bill, tax bill, rental agreement etc.)

  2. Student's Birth Certificate

  3. Student's Immunization Record

If you are new to the district, a meeting with our principal will be set-up when you bring in the paperwork to the Guidance Office.
